Servicing and repairs of medical devices, under warranty or not, acquired
from Amertek Medical, Inc.
and Alton Design, LLC will be arranged directly through Micro Tool Engineering,
Inc.
First, call Micro Tool Engineering, Inc. (Dinah) at (561) 842-7381
to get the necessary information
and to ascertain that we can perform the servicing and repairs in time for
your next procedure.
A fax or email will be sent to you outlining the procedure to be followed,
together with a
"Repairs and Servicing Form". This form is also available, in PDF format,
for printing (Click Here).
Ship the items, together with the completed Repairs and Servicing Form to
us as per the
information provided to you in the fax or email. We will then perform the
repairs and
servicing as per the device specifications and returned it to you as previously
agreed.
